Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to crushing and fire/explosion hazards:
(a)On or before February 8, 1994, at 3838 N. 59th Street in Kansas City,
Kansas, an employee was pumping liquid propane from a 24,000 pound rated
Kodiak
bobtail vehicle #5379 parked on a residential driveway.  The employee was
exposed
to an explosion/fire and crushing hazard created by the placement of only
one wheel
chock block, which was also of inadequate size, to prevent the propane
truck from
rolling uncontrolled, striking any object in its path.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is at a minimum consider the following:
1.Ensure that all trucks leaving the facility have the required number of
chocks
as required by the National Fire Protection Association #58, Section 3.15.
2.Ensure all truck operators are trained in the procedures regarding the
placement of chocks.
3.Ensure wheel chocks used meets the criteria set forth in the Society of
Automotive Engineers Standard J348

employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of being struck by a rolling propane
vehicle:
(a)On or before February 8, 1994, at 3838 N. 59th Street in Kansas City,
Kansas, an employee was pumping liquid propane from his 24,000 pound rated
Kodiak bobtail vehicle #5379 to a residential 500 gallon tank.  Vehicle
#5379 was
parked on an inclined gravel driveway with the engine running exposing an
employee
to a hazard of being struck by a rolling vehicle as a result of a
non-functioning
emergency parking brake.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is at a minimum, the following:
1.Conduct daily inspections with written reports of vehicles and assure
all unsafe
conditions are corrected prior to the vehicle leaving the establishment.
Daily
inspections will consist of the following:  Lights, reflectors,
suspension, tires,
wheels, rims, lugs, battery, exhaust, brakes, rear end protection, warning
indicators, windshield wipes/washers, horns, heaters/defrosters, mirrors,
steering, clutch, service/parking/emergency brakes, triangles, fire
extinguisher,
spare fuses, seat belts, other safety equipment, oil/coolant level, belts,
grease/oil/fuel/coolant leaks, body/doors, cab/doors/windows.
2.Ensure that all employees have received adequate training in the safe
operation
and deliveries of liquid
