Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards from steel rack units and/or its contents and from an automotive lift: The following equipment used by the associates at the workplace were not maintained in accordance manufacture or national consciences standards. Associates were exposed to crushing hazards: a)Dept 1512 - Calendar Wire Storage Area, the racks which are supporting rolls averaging 3500 pounds and in all other areas where damaged racks are located, the employer did not ensure that steel storage racks in use at the facility were inspected and maintained in accordance with the manufacturer's requirements and with the American National Standards Institute guidelines for the use of industrial and commercial steel storage racks. Among other methods, one feasible and acceptable method to correct this hazard is to follow the recommendations of the Rack Manufacturers Institute Specifications for the Design, Testing, and Utilization of Industrial Steel Storage Racks 1995 Edition, Section 1.4.1: Develop and implement a structural integrity program to periodically inspect and repair storage racks. b)Facilities - the automotive lift was not equipped with a manual safety locking device which will prevent downward movement of more than six inches after stopping motion. There are manual safety locking devices (safety latches) which can be engaged after fully elevated. Employees were exposed to crushing hazards. Among other methods, one feasible and acceptable method to correct this hazard is to follow the recommendations of ANSI/ALI ALCTV-1998 Standard for Automotive Lifts- Safety requirements for construction, testing and validation; ALI SMO1-1 Lifting it Right A Safety Manual from the Automotive Lift Institute; ANSI/ALI ALIS-2001 Standard for Automotive Lifts Safety Requirements for Installation and Service: 1. Install or repair automatic mechanical safety locking device on lift. 2. Instruct and train employees in the proper use of mechanical locking device for hydraulic lifts. 3. Conduct periodic inspections of the lift as recommended by the lift manufacturer, to insure that the safety locking device and all other components are in safe operating condition. 4. Post manufacturers operating manual in the service area for reference. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is completed.

Section 5(a)1 of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to below the hook attachment hazards: The followng below-the-hook attachments developed and built in house was not load rated or load tested. The attachments were not marked with a load rating. Associates were exposed to struck by hazards. a. Dept 5330 - Wind-Up Lift Reel - lifting device for 300 pound rolls b. Hoist #830 -Fabric Counter - lifting device for lifting 300 pound rolls c. Dept 1414 - Band Building 01-02 - lifting device for 150 pound rolls d. Dept 1512 - EMIO6N - lifting device for 300 pound rolls e. Plant 2 - Clamp Rings, lifting device with Vise Grip clamps. Feasible and acceptable methods among other methods to correct the hazard include but are not limited to following the recommendations is to follow Below-the-Hook Lifting Devices, ASME B30.20-1993. 1. Provide the rated load on any custom made lifting device. 2. Design the lifting device to withstand the forces imposed by its rated load with minimum design factor of three. 3. Provide initial, frequent and periodic inspections of the lifting devices. 4. Ensure that the initial proof testing is 125of the rated load was conducted. 29 CFR 1903.19(d)(1) requires certification of the above violation is completed.
