Section 5(a)1 of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to below-the-hook attachments:
Converting Area - the below-the-hook attachments developed and built in
house were not load
rated or load tested.  The attachments were not marked with a load rating.
Employees are
exposed to stuck by hazards.
Feasible and acceptable methods to correct this hazard is will include but
not be limited
to: 1. Provide the rated load on any custom made lifting device.  2.
Design the lifting device
to withstand the forces imposed by its rated load with minimum desgn
factor of 3.  3. Provide
initial frequent and periodic inspections of the lifting devices.  4.
Ensure that initial proof testing
of 125of the rated load was conducted.  ANSI B30.20 "Below the Hook
Lifting
Devices".
29 CFR 1910.19(c)(2) abatement of the above violation was verified at the
time of
inspection.  No certification is required.
