Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of falling from the bucket loader and being struck or run over: JOBSITE: AN EMPLOYEE WAS OBSERVED RIDING ON THE SIDE OF A FRONT END BUCKET LOADER. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO PROHIBIT ALL EMPLOYEES FROM RIDING ON ANY PART OF THE BUCKET LOADER.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by broken chain or by falling objects such as, but not limited to, 12" ducktile metal pipe and/or machinery being hoisted by non alloy chain: JOBSITE: IN THE FOLLOWING LOCATIONS CHAINS, OTHER THAN STEEL ALLOY, WERE USED TO HOIST SMALL MACHINERY AND 12" DUCKTILE IRON PIPE. A. KINNE ROAD, 12" DUCKTILE WATER PIPE, & 12" DUCKTILE WATER PIPE WITH A FIRE HYDRANT ATTACHED TO IT. B. UPLANDS WAY, 12" DUCKTILE WATER PIPE. C. ALONG NEW LONDON TURNPIKE, BACKHOE WITH STREET SWEEPER ATTACHED TO THE FRONT OF THE MACHINE. D. SECTION OF NON ALLOY CHAIN ATTACHED TO THE FRONT OF THE FRONT END BUCKET LOADER. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS": (a) TO USE A STEEL ALLOY CHAIN, IN ACCORDANCE WITH CFR 1910.184 WHICH IS THE ONLY INDUSTRY RECOGNIZED METHOD FOR HOISTING. (b) TO TRAIN EMPLOYEES PERFORMING HOISTING OPERATIONS ON SAFE OPERATING PROCEDURES SUCH AS HOW TO RECOGNIZE A STEEL ALLOY CHAIN. A STEEL ALLOY CHAIN WILL HAVE A PERMANENTLY AFFIXED DURABLE IDENTIFICATION STATING SIZE, GRADE, RATED CAPACITY, AND MANUFACTURER. SPECIFIC ABATEMENT DOCUMENTATION IS REQUIRED
