Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: WAREHOUSE: A CARABINEER HOOK CONNECTOR, USED TO ATTACH A SELF- RETRACTING LIFE LINE (SRL) TO AN ANCHORAGE, WAS BENT. THE DEFLECTION OF THE BEND PREVENTED THE CONNECTOR FROM LOCKING. A LOCKING CARABINEER IS REQUIRED BY ANSI Z359.1-1992 TO PREVENT "ROLL-OUT." ONE FEASIBLE ABATEMENT METHOD, AMONG OTHERS, IS TO REPLACE THE DAMAGED EQUIPMENT.
