recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to multiple injuries from using electrical equipment that was
damaged and did not
operate properly , in conjunction with the lack of frequent and periodic
inspections being
performed on the jib cranes and overhead hoists:
(a)Shop Area; 220B Ferry Station - The controls for the electrical
overhead CM
Lodestar hoist were not operating properly in that instead of lifting the
load, the
hoist would sometimes lower the load.
(b)Shop Area - Frequent and periodic inspections on the jib cranes and
overhead
hoists were not conducted on a daily to monthly basis.
"AMONG OTHER METHODS, FEASIBLE AND ACCEPTABLE ABATEMENTS METHODS
TO CORRECT THE HAZARDS ARE TO:"
1.Ensure that any equipment in disrepair or not functioning properly are
immediately reported and promptly adjusted or repaired before operations
are in
process.
2.Implement an inspection program for the jib cranes and maintain monthly
records
in accordance with the American National Standard Institute (ANSI)
standard on
"Monorail Systems and Underhung Cranes" (ANSI B30.11-1988) and "Overhead
Hoists" (ANSI B30.16-1973).
