Section 5(a)(1) of the Occupational Safety and Health Act of 1970;  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a falling hazard from the unsecured platform;
(a)Sid Tool Company Site - Sky Trak machines were used to elevate
personnel on
platforms resting on the machines' lifting carriages and/or forks to
heights of up
to approximately thirty one (31) feet above the ground.  The platforms
were not
secured to the lifting carriages and/or forks while employees worked on the
platforms.  This condition exposed employees to a fall hazard between
April 22
and May 1, 1996.
AMONG OTHERS, ONE FEASIBLE AND ACCEPTABLE METHOD TO
CORRECT THIS HAZARD IS TO SECURE THE PLATFORM(S) TO THE
LIFTING CARRIAGE AND/OR FORKS.

employees were exposed to:
(a)Sid Tool Company Site - Two fiberglass portable extension ladder were
set on a five (5)
feet high sections of scaffold and were used to reach the roof area above.
This condition
exposed employees to a fall hazard of thirty five (35) feet to the ground
below during the
period between April 22 and May 1, 1996.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS TO INSTALL A LADDER OF THE
APPROPRIATE LENGTH TO ALLOW EMPLOYEES TO SAFELY REACH THE
WORK LEVEL.
