Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of electrocution. (a) On or about May 3, 1994 an employee of Smith Dairy located at 6345 County Rd. 33, Yuma, CO. was fatality electrocuted while using a 120 volt 1/2" drive Milwaukee brand drill to install a fence next to the "maternity barn". Investigation results conclude that the wiring configuration on the service panel had it's polarity reversed. A feasible and acceptable abatement method to correct this hazard is to wire the service panel per the National Electrical Code book as indicated in paragraph 200-11.

(a) On or about May 3, 1994 an employee of Smith Dairy located at 6345 County Rd. 33, Yuma, CO. was fatality electrocuted while using a 120 volt 1/2 drive Milwaukee brand drill to install a fence next to the "maternity barn". Investigation results conclude that an unused portion of the metal electrical box was not enclosed in that a "knockout" was missing. A feasible way of abating this hazard is to follow the National Electrical Code book, paragraph 370-18 or 11012(a) and effectively close all unused portions of electrical boxes.

(a) Location, Smith Dairy Farm, 6345 County Rd. 33, Yuma CO. 80259 - On or about May 3, 1994, an employee of Smith Dairy Farm was fatality electrocuted while installing a new fence next to the maternity barn. Investigation results concluded that electrical system was not properly grounded in that the grounding wire was energized with 124 volts. One feasible and acceptable abatement method to correct this hazard is to properly ground the electrical system per the National Electrical Code Handbook listed in paragraph(s) 250-42 and 250-114.

(a) Location, Smith Dairy Farm. Yuma, CO. - On May 3, 1994 an employee of Smith Dairy was fatality electrocuted while using a "Milwaukee" brand electric drill to install a fence next to the maternity barn. The outer jacket on the electric drill was frayed subjecting the insulated conductors to possible damage and moisture. One method of abating this hazard is to follow the National Electrical Code book listed under article 110-3 and inspect portable electric tools prior to using.

(a) Location: Smith Dairy Farm, 6345 County Rd. 33, Ymua CO. 80259, On or about May 3, 1994, an employee was fatality electrocuted while installing a new fence/gate next to the maternity barn. The non metallic sheathed cable (romex) leading into the conduit approximately two feet above the electrical box was not adequately closed. An acceptable way of abating this hazard would be to install the electrical system per the National Electrical Code book listed in article 370-17(a) and close the opening where the "romex" enters the conduit.
