employer did not

recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to: crushing hazard by the collapse of the steel I-beams that
support the tail pulley
and the walkway.
(a)Stock Prep Area, Number One (1) Tread Line - The support I-beams for the
walkway and tail pulley on the number one (1) tread cooling belt at the
north end
were showing severe deterioration due to rust.  The beams were reduced in
size
from three eights (3/8) inch thickness to one quarter (1/4) and one thirty
second
(1/32) of a inch.  The beams at certain areas crumbled under a finger
touch.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD IS TO HAVE THE STRUCTURE EVALUATED BY A REGISTERED ENGINEER
AND APPROPRIATE CORRECTIONS MADE TO STRENGTHEN THE STRUCTURE.

a
sixty six (66) inch long spreader bar that was attached to an underhung
crane
equipped with a two (2) ton electric Budget hoist.  The spreader bar was
not
marked for the rated capacity and employees were lifting 2240 pounds with
the
spreader bar.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS TO RATE THE CAPACITY OF THE
SPREADER BAR AND MARK IT ON THE SIDE.
