employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to: crushing of the hands and
fingers from unexpected operation of the press.
(a) Machine Shop Area - The Cincinnati 300 ton, Chicago 100 ton, Chicago
25 ton and the
Diacro 20 ton press brakes did not have their foot treadles guarded to
prevent accidental
tripping, on or about May 1, 2006.
"AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO
CORRECT THIS HAZARD IS TO PROVIDE AND USE FOOT TREADLE GUARDS FOR THE
PRESSES"

to a crushing hazard from the
three ton underhung bridge crane with a pendent operated underhung trolley
and hoist that was installed with
no prior engineering evaluation and approval, nor was the welding
preformed in accordance with ANSI/AWS
D14.1:
(a)Welding and Shipping Bays - Underhung single bridge with an underhung
pendent operated
trolley and hoist, manufactured by Cleveland Tramrail crane used to lift
1200 pounds, was
installed with no prior professional engineering evaluation approval
concerning the crane
runways, monorail, and supporting structures that they were designed to
withstand the loads
and forces imposed by the cranes and carriers. There was no indication
that the  welding used
on load-sustaining members of the crane conformed to ANSI/AWS D14.1,  on
or about May
1, 2006.
"AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD
TO CORRECT THIS HAZARD IS TO HAVE A PROFESSIONAL ENGINEER EVALUATE THE
INSTALLATION OF THE CRANE AND COMPONENTS AND DETERMINE THAT THE CRANE
CAN WITHSTAND THE LOADS AND FORCES IMPOSED UPON IT, AND ASSURE THAT THE
WELDING FOR THE LOAD SUSTAINING MEMBERS OF THE CRANE DOES CONFORM TO
ANSI/AWS D14.1."
