Section 5(a)(1) of the Occupational Safety and Health Act of 1970. The
employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to potential fire and dust
explosion hazards because methods and practices were not utilized to
properly design dust collectors to prevent
explosions and prevent propagation of a fire/explosion into occupied area:
(a)Outside Cabinet Shop - The Norfab dust collector was not provided with
deflagration vent,
backflash damper and high speed abort gate to minimize pressure
development in the event of
an explosion, on or about May 7, 2008.
(b)Outside Cainet Shop - The Norfab collector system was not grounded and
bonded, on or about
May 7, 2008.
(c)Cabinet Shop, Frame Department - The Dantherm dust collection system
had a capacity of
over 5000 cfm and located inside the building, on or about May 7, 2008.
(d)Cabinet Shop, Frame Department - The Dantherm duct collector system was
not grounded and
bonded, on or about May 7, 2008.
ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement
method to
correct these hazards is to comply with the National Fire Protection
Association (NFPA 664)
"Standards for the Prevention of Fires and Explosion Processing and
Woodworking Facilities" 2007
edition as it applies to your equipment including but not limited to the
following:
* Recycle exhaust air back into the building from dust collector in
accordance to 8.2.2.6
* Locate dust collection equipment in accordance with section 8.2.2.5.1
* Ductwork shall be grounded and bonded in accordance to 7.9.2.
