Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employees a
place of employment which were free from recognized hazards that were
causing or likely to cause death or
serious physical harm to employees, in that employees were exposed to
falling from an elevated homemade
forklift platform:
(a)Metal Rack Area, Berkeley Contract Packaging, LLC, 597 Alexander Spring
Road, Carlisle
PA - The homemade wooden work platform, used in conjunction with a
stand-up forklift, was
not approved or rated for its intended purpose, such as elevating
personnel to perform
inventory duties, thus exposing employee(s) to a possible fall hazard, on
or about July 21,
2004.
ABATEMENT NOTE:
Among other methods, one feasible and acceptable abatement method to
correct this hazard to utilize
safety platforms that meet the requirements of American Society of
Mechanical Engineers, ASME
B56.1-2000, Safety Standard for Low Lift And High Lift Trucks.
