Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: crushing from the failure of the homemade lifting device that is attached to the hook: (a)Molding Department, Big Sinto Molding Machine - The "homemade" spreader bar that was attached to the one ton jib crane used to lift patterns did not have a capacity label on the device, on or about March 11, 2011. (b)Cleaning and Finishing Department - The "L" type "homemade" hook attached to the one ton under hung jib hoist did not have the capacity marked on it, on or about March 11, 2011. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO TEST AND CERTIFY THE RATED CAPACITY OF THE HOMEMADE LIFTING DEVICES AND MARK THE CAPACITY ON THE DEVICES AS REQUIRED BY ASME B30.20 - 2003 BELOW-THE-HOOK LIFTING DEVICES" VERIFICATION REQUIRED
