Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: crushing injuries caused by the collapse of 42 foot span truss. a.Arthur Ave Clarksburg, WV - The last truss placed on the North end of the building was not supported by temporary ground bracing to the exterior of the building on or about 11-29-01. b.Arthur Ave Clarksburg, WV - The first truss that was installed in the middle of the building and working to the North, was not supported by temporary ground bracing to the interior of the building on or about 11-29-01. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS ..."TO INSTALL ADEQUATE TEMPORARY GROUND BRACING TO THE INTERIOR AND EXTERIOR OF THE BUILDING IN ACCORDANCE WITH THE TRUSS PLATE "HIB-91 SUMMARY SHEET RECOMMENDATIONS FOR HANDLING, INSTALLING AND BRACING WOOD TRUSS.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: crushing injuries caused by the collapse of 42 foot span truss. a.Arthur Ave, Clarksburg, WV - There was only (2) two continuous top cord lateral braces on a 42 foot span wood truss, starting at 31 inches up from the eave and continue up the 271 inch long cord to the peak, spacing (10) ten foot apart with a peak brace that alternated from the east to the west side of the peak o or about 11-30-01 . b.Arthur Ave, Clarksburg, WV - There was no lateral or diagonal bracing in place on the bottom cord of the truss on or about 11-30-01. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS ..." INSTALL THE MINIMUM AMOUNT OF LATERAL BRACING RECOMMENDED BY TRUSS PLATE INSTITUTE HIB - 91 SUMMARY SHEET, RECOMMENDATIONS FOR INSTALLING AND BRACING WOOD TRUSS, FOR A 42 FOOT SPAN TRUSS, WHICH THERE WOULD HAVE BEEN (5) FIVE CONTINUOUS TOP CORD LATERAL BRACES SPACED 6 FEET APART, FROM THE EAVE OF THE TRUSS TO THE PEAK. INSTALL BOTTOM CORD DIAGONAL AND LATERAL BRACING.
