Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries: (a)Machine Shop. Less than two wraps of rope remained on the drum when the hook of the Robbins & Myers hoist was in the extreme lower position. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS to install a sufficient amount of rope on the drum so that no less than two wraps remain on the drum with the loaded hook at its extreme lower position or, install a lower limit device in which case no less than one wrap shall remain.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries: (a)Middle Crane. The Euclid underhung crane was in operation without a limit switch. (b)Machine Shop. Adjustments were not made to the limit switch on the Robbins & Myers underhung crane, located next to the freight elevator, to assure correct functioning. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS to inspect the cranes and correct any unsafe conditions disclosed by the inspection before operation of the crane is resumed.

AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS to conduct frequent (daily to monthly interval) and periodic (one to twelve month interval) inspections of critical crane/hoist components to determine the degree of exposure to wear, deterioration or malfunction. Written, dated, and signed inspection reports/records should be made on critical items such as hooks, brakes, ropes, chains, and safety devices.
