Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: A fall hazard of 120 feet while two employees were riding the load (gin pole) as it was being lifted to the top of the existing telecommunications tower by the use of a Clyde Frame 4, three drum hoist, which was setting unsecured on a flat bed trailer on a 10 degree slope, 75 feet from the base of the tower; of which the hoist slid on the trailer and dropped the gin pole and employees to the ground. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS ..." Do not permit employees to ride the load

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: the Clyde Frame 4 hoist slipping on the flat bed trailer and dropping the load of a 1,200 pound gin pole, of which two employees were riding, while attempting to raise the gin pole into position at the top of the existing 120 foot high telecommunication tower ; the hoist was not secured to the trailer nor was the trailer secured to a fixed object while setting on a 10 degree slope 75 feet from the base of the tower. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS ..." to secure the hoist and trailer prior to attempting a lift.
