employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the recognized hazard of being struck by falling portions
of the hoisted load or
the hoist of the Shepherd Niles 5 ton underhung crane located in bay #1,
westside of the facility,
south crane on this runway.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to properly maintain all chain hoist hooks in accordance with Paragraph
11-2.3.3(c)(2) of the
American National Standards Institute (ANSI) B30.11-1973, Monorail Systems
and Underhung
Cranes.

and assure that at the beginning of each work shift, each employee test
the hoist limit switch on
the underhung crane that they will be utilizing, in accordance with
Paragraph 11-3.2.4(a) of the
American National Standards Institute (ANSI) B30.11-1973, Monorail System
and Underhung
Cranes.
