Warehouse:
1)  The car gate on the freight elevator was damaged and would not close
exposing the
emloyee(s) to serious injuries.
2)  The opening in the basement shaftway door was not guarded.
One method, among other methods, to correct the condition is to replace
the elevator car
gate and guard the opening in the basement shaftway door.way
