Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employmnent a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to serious burns or death:
Master Drive; Yard:
Employees were exposed to serious burns and explosion hazards
when lighting the pilot lights of the two 3000 pound
thermoplastic
heaters.  The safety devices, the pilot regulators and
the thermostats, were capable of being bypassed through a ball
valve that allowed the propane gas to enter directly into the
burner units in violation of NFPA 58 3-8.4.2 (b).  Also the
piping for the propane located on the sides of the burner
units were not protected against physical damage as required
by NFPA 58 3-8.2.7 (e).
One feasible method, among other methods, to correct the hazards is
to remove the bypass line and protect the piping against physical
damage.  Also employees required to light the burner units and
those exposed to the molten thermo-plastic shall be required to
wear face shields and shall wear cotton clothing during operations.
