Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employmnent and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were not provided with fall protection and were exposed to serious fall
injuries while climbing
on the standard railing of the unsecured mobile ladder to gain access to
the top of a 15 foot
fiberglass tank.
Crane Department:
Employee(s) were not provided with fall protection and were exposed to
serious injuries
while climbing on the standard railing of the mobile Hi-Jacker stairway to
gain access
to the top of the 15 foot tank to remove the hook from the center beam.
Feasible and useful methods to correct the hazards among other methods are
as follows:
1) Provide the employee required to remove the hook from the center beam
of
upright
tank(s) with fall protection and safety harness.
2)Instruct all employees to use the two anchoring devices located in front
of the two rear
caster wheels on each side of the mobile Hi-Jacker stairway to ensure that
the unit is
stable prior to use.
3)Employees shall be required to elevate the mobile Hi-Jacker stairway to
the height of the
tank which will preclude the need to climb the standard railing to gain
access to the top
of the tank.
