Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a) As referenced in ANSI B20.1-1976 Safety Standards for Conveyors and Related Equipment, section 5.09.2.1 Employees exposed to the hazards created by an unguarded conveyor belt, located on the north side of Koppers die cutter, at trash suction vent.. Feasible Method of Abatement is: 1) Install a guard across opening to conveyor belt, trash suction vent, and die cuter. The guard will prevent employee access during operation of machinery.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a) As referenced in ANSI B20.1-1976 Safety Standards for Conveyors and Related Equipment, section 6.11.1.2 Roller and Wheel Conveyors: Personnell should not be allowed to walk or step on free turning rollers or wheels. Employee(s) exposed to the hazard of falling onto operating C & M Conveyor (roller type), s/n not legible. While walking on conveyor system in the performance of job duties after corrugated boxes come out of printer, located in the Fabrication Area. Feasible Method of Abatement is: 1) Suitable means such as tread plates can be used between the rollers as a walking surface for designated operator in the performance of job duties.
