Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Site (Lock 17 Road - intersection of Paul Almond Road, Oak Grove, Alabama), Vehicle (Workover Rig #1) - On or about 9/30/96 the workover rig # 1 was operating equipped with seat belts that were installed improperly, in that two male ends were in place with no female end, making them incapable of use and creating a risk of injury due to the lack of an effective operator restraint system.
