Broadway, North Little Rock, AR 72114, employees were exposed to hazards posed by not being secured in the seating compartment by forklift equipped seat belts in the event of forklift tip over. (e.g.,crush and struck by type injuries from the falling object protective structure (FOPS) and/or other parts of the greater than 4000 pound forklift.
