Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to a struck by hazard:
a.Employees were not sufficiently trained to follow the company's Blowout
Prevention Control Procedures in that four employees on Rig #7 were exposed
to the hazards of being struck by a configuration of a sudden, violent
released
of pressurized air from the gas well and the blowout preventer, and (it's
believed) by part of the flowline that was connected to the airbowl of the
Blowout Preventer.  The employees did not control the sudden, violently
release of pressure by immediately closing off the blowout preventer,
before
working on the flowline, as specified in Chapter I, Section F, Sub-section
1c,
of the company (The Jack Company and the S.W. Jack Drilling Company)
safety manual.  Among other methods, one feasible and acceptable abatement
method to correct this hazard would be to establish and implement
sufficient
training which would properly and adequately prepare employees to recognize
and respond to unusual drilling and well control situations, therefore
prohibiting carless acts such as , but not limited to, employees not
closing
down the blowout preventer, while working on a clogged flowline on a hole
that was under extremely high pressure, as specified by Chapter I, Section
F,
Sub-section 1c, page 15 of the company (The Jack Company and the S.W.
Jack Drilling Company) safety manual.  This training should require
continuous observation of all employees work performance in a safe and
effective manner.   This alleged condition occurred on September 23, 1998.
b.Four employees on Rig #7 were exposed to the hazards of being struck by a
configuration of a sudden, violent released of pressurized air from the
gas well
and the blowout preventer, and (it's believed by part of the flowline that
was
connected to the airbowl of the Blowout Preventer, in that the employees
did
not control the sudden, violently release of pressure by immediately
closing off
the blowout preventer, before working on the flowline, as specified in
Chapter
1, Section F, Sub-section 1c, of the company (The Jack Company and the
S.W. Jack Drilling Company) safety manual.  Among other methods, one
feasible and acceptance abatement method to correct this hazard would be to
ensure that the Blowout Preventer is closed before working on the flowline.
