GENERAL 5A1 WORDING Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a)The chute system designed to support 2,000 pounds including it's own weight, used to send scrap roofing material from the roof area to the truck approximately 60 feet below, was overloaded with scrap roofing material.. "AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS ..." Assign a ground person to monitor the chute and assure that material is emptying from the bottom chute into the back of the truck not backing up in the chute. (b)Employees using a chaeta system attached to the side walls of the building to move waste roofing material from the roof area to the ground level truck, did not follow manufacturers safety instructions in that they allowed excessive waste roofing material to accumulate inside eight of the twenty chutes due to the back of the truck being loaded with material which blocked the exit of material from the bottom chute. " AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS..." Assign a person to monitor the chutes to assure there is no backup of material in the chute. (c)The employer did not follow the manufacturers safety instructions of never discarding debris in the chute that is larger than 1/2 the size of the chute in that roofing material of various sizes of 17 x 20 inches up to 23 x 19 inches was discarded in the chute that was 24 inches in diameter. " AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS..." Allow only roofing material of a size that is 1/2 the size of the chute to be discarded in the chute.ded
