Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to 50 foot wide truss being repaired that had
previously collapsed.
The employer was repairing the 2 x 6 truss with various types of material
such as but not
limited to 2 x 4's and press board scabs, and some truss were completely
disassembled and
rebuilt, without the approval of the truss design engineer and
manufacturer.
"AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT
METHOD TO CORRECT THIS HAZARD IS ..."To Follow the recommendations in HIB-
91 Appendix B and have any repair approved by the truss design engineer.

span truss that
was being set in place by the use of a crane. The crane was connected to
the truss peak with
only one lifting device allowing  for an unstable load and the truss to
swing freely in the air.

METHOD TO CORRECT THIS HAZARD IS TO FOLLOW THE RECOMMENDATIONS
IN HIB-91COMMENTARY AND RECOMMENDATIONS FOR HANDLING,
INSTALLING AND BRACING METAL PLATE CONNECTED TRUSS.

employees were exposed to crushing injuries caused by the collapse of
inadequately braced
50 foot span truss.  The contractor did not follow the bracing
instructions provided to him by
the truss designer / fabricator for lateral and diagonal bracing on the
top and bottom cord of
the truss, ground bracing on the building exterior on the gable end and
failed to provide the
required number of temporary bracing specified for the 50 foot span truss.

METHOD TO CORRECT THIS HAZARD IS TO FOLLOW THE RECOMMENDATIONS
