recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
At the construction of the new parking deck located on the ASU campus,
located at 111
North Caraway in Jonesboro, the operators of the Sky Trak rough terrain
high lift fork
trucks were observed traveling throughout the area with their seat belts
unbuckled.  This
practice exposed the operators to the hazard of being ejected and crushed
by the machine
in a roll-over accident.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to require the operators to wear their seat belt at all times
when he is sitting
in the seat of the powered industrial truck, regardless of the length of
time
that they will
be in the seat, as warned by the manufacturer of the lift in a warning
posted on the frame
of the lift.
