Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to: the hazard of falling from a height.
On site at the Reynolds overpass on the west bound service road of
Interstate 30, beneath
the bridge where carpenters were working from the platform elevated by the
SkyTrac
Model 10042, Rough Terrain powered industrial truck. Employees rendered
the guard
rail system of the platform ineffective by standing on the rails to
achieve a greater height.
The employees were wearing harnesses and lanyards, but the lanyards were
not "tied off"
to any support. The ASME B56.6-1992 Safety Standard for Rough Terrain
Forklift
Trucks in Section 8.25.1(l) requires that the body belt and lanyard is to
have an
attachment point provided overhead for freedom of movement.
A feasible abatement for this hazard is to provide single and/or double
"dee" ring beam
clamps that affix to the bottom of the bridge I-beams or a "dee" ring
assembly that bolts
into holes drilled in the I-beams.

that employees were
exposed to: the hazard of a fall from height.
At the worksite located beneath the Reynolds Road overpass, west bound
service road;
Bryant, Arkansas; where employees were observed working in a basket,
elevated
approximately 10 feet high above the ground by a SkyTrac model 10042 Rough
Terrain
forklift, that had a broken front midrail. This condition exposed the
workers to the
hazard of a fall from the platform to the pavement below. The ASME
B56.6-1992
Safety
Standard for Rough Terrain forklift trucks, in Section 8.25.1(l) requires
midrails to be
provided on elevatable platforms. Feasible abatement is to replace the
damaged midrails
with new steel tubing welded in place.
The employer was previously cited for a violation of this Occupational
Safety and
Health Standard which was contained in OSHA inspection number 303644389,
Citation number 1 Item number 2, issued on December 11, 2001, with respect
to a
workplace located at S H-4 at SW 149th Street, Tuttle, Oklahoma.
