Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: TELECOMMUNICATION TOWER CONSTRUCTION SITE, 5650 W. LIBERTY AVE., HUBBARD, OHIO: Tuesday, December 21, 1999, the employees, who were working on the job site, were exposed to the hazard of being struck-by materials and equipment due to failing crane components. The employer did not ensure that the truck-mounted crane was equipped with the appropriate length of boom for the operation. While hoisting cross-braces to the top of the forty-foot section of tower, the crane hook contacted the load block exposing the crane components to potential damage and possible failure. Among other methods, two feasible and acceptable methods to control the hazard are; (1) Utilize a truck mounted crane with the appropriate boom height to prevent the hook from contacting the load block; (2) Utilize an anti-two-blocking device or a two-block damage prevention feature that deactivates the hoisting action before damage occurs.
