Section 5(a)(1) of the Occupational Safety and Health Act of 1970: %% The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: At the job site, circuits in which employees were installing light fixtures were not tested to determine de-energization. Among the feasible means of abatement are: 1. Testing equipment should be used by the employer to ensure total de-energization before work is performed.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: At the site, where light fixtures were being replaced, the employer did not require nor enforce the use of locks and /or tags to ensure that circuits were deenergized (sometimes black tape was placed across the breaker switches) thereby exposing employees to an electrical hazard. Among the feasible means are: 1. Locks or Tags should be applied to ensure the unexpected startup of energy.ted
