Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were causing or
likely to cause death or serious physical harm to employees in that
employees were exposed to:
a) The hazard of being struck by falling rolls of paper due to failure of
hoist systems used to lift
paper rolls. Manual chain hoists at the north and south sides of # 1 and #
2 paper mills, along
with their supporting structures, were not inspected at sufficient
intervals, based on the severity
of service and the environment. Inspection intervals were not sufficient
to detect new deficiencies
as they occurred or to monitor further deterioration of existing defects.
Timely repairs were not
made for defects discovered several months earlier on the manual chain
hoists, and their
supporting structures, at the # 1 and # 2 paper mills and at the east
loading dock.
Among other methods, one feasible and acceptable abatement method to
correct
this hazard is to
conduct regular and recurring inspections and to perform subsequent repair
and maintenance
during scheduled shutdown periods.
