Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to:
On and prior to 7/10/95, employees conducting set-up operations on
a Verson power press brake, which is used for die cutting
operations, were exposed to point of operation hazards.
Among other methods, the following are feasible and acceptable
methods of minimizing the probability of employee injury from point
of operation exposure.
(1) Establish written die-setting procedures for power press brakes
which include instructions that would strictly limit die set-up
operations to one employee, and include instructions for the
removal of the run/inch/continuous selector key after the setup is
complete. The key shall be kept by the supervisor.
(2) Train all effected employees on the requirements set forth in
the procedures, and in the OSHA and ANSI standards which apply, and
any special instructions from the equipment manufacturer. The
minimum training shall include:
a) A description and identification of the hazards associated
with the operation of the press brakes.
b) How and why the safeguard(s)s are used, how they provide
protection, and the hazard for which they were intended.
c) How and under what circumstances safeguards can be removed or
disabled and by whom.
d) What to do if a safeguard is damaged, missing, inoperative,
or unable to provide adequate protection.
