Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
On or about June 17, 1993, during the erection of a two story office
building at Rhodes Road and State Route 59 in Kent, Ohio, employees were
exposed to the hazards of serious injuries such as lacerations, broken
bones, or death from a roof collapse while installing metal plate
connected
wood trusses approximately 76 feet long on top of 10 feet high wood
bearing
walls.  Inadequate lateral bracing and inadequate top chord diagonal
bracing were used for the truss erection.
The hazards can be abated by, but not limited to, the following:
(a)  Establish proper spacing of both lateral and top chord diagonal
bracing by use of a registered professional engineer for trusses over 60'
in length as recommended by the Truss Plate Institue and/or by utilization
of truss manufacturers recommendations during erection of trusses.
(b)  Utilize proper lifting practices for truss length over 60' per Truss
Plate Institute, or per gang nail warning literature provided with trusses
during delivery.
(c)  Assure permanent bracing is installed prior to placing loads of OSB
(Oriented Strand Board) or piggy back cap trusses on top of installed
trusses.
