Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
Employees tending the scrap furnace were exposed to the potential hazard
of molten metal
eruption / explosion during charging operations due to inadequate
procedures to prevent wheels
containing moisture, grease, or other foriegn meterial from being charged
into the molten metal
bath of the furnace. Procedures were inadequate in that:
A. Scrap wheels were stored such that the wheels last exposed to wet
processes
were potentially
the first wheels to be charged, and therefor more likely to be wet.
B. Wheels were inspected for moisture or foreign material on one side
only. Procedures did not
require the wheels to be turned over, in order to thoroughly inspect both
sides.
C. Procedures for introducing wheels into the molten metal bath via roller
conveyor did not
ensure that all wheels were sufficiently pre heated prior to being pushed
into the bath.
Among other methods, one feasible and acceptable method to correct this
hazard would be to
implement procedures including, but not limited to, the following:
A. Institute a procedure to ensure that the wheels last exposed to wet
processes would be the last
wheels taken from the scrap storage area, allowing more time to dry.
B. Institute inspection procedures that would ensure that both sides of
the wheels were inspected
for moisture or foreign material prior to charging them into the bath.
C. Institute a procedure that would allow a sufficient pre heat time for
each wheel at the edge
of the molten metal bath, prior to charging them into the bath.
