Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a)On or about April 18, 1999, employees were exposed to the hazard of being struck by flying metal parts while performing a deburring operation on a metal shaft on lathe #07603-010, located in the maintenance shop at the unsafe spindle speed of 2000 RPM. Among other methods, one feasible method to correct this hazard is to adequately train employees on the proper and safe spindle speeds for various junctions performed on lathes, in accordance with ANSI B11.6 safety requirements for the construction, care, and use of lathes.
