Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: Employees working in the vicinity of the hoists were exposed to the hazards of falling loads, hoists, and their components due to the following conditions: (a)Hoist located in Bay #4 had the following defects: The start and stop buttons on the pendant control, when depressed, caused the hoist to drift and the brake to not hold. The pendant had missing screws, leaving openings. (b)Hoist located in the bay #3 had no identification markings on the pendant and the pendant cover screws were missing leaving openings. (c)On or about 11-25-97, the hoist located in Bay #4 was being operated with wire rope cable which was not of sufficient length to maintain 2 wraps on the drum.. One feasible and acceptable method, among others, to correct these hazards is: Establishing a preventative maintenance program, which includes conducting frequent and periodic inspections by a competent person on all hoists and hoist components and support structures with corrective measures taken as needed.
