Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm in that
employees were exposed
on or about early October, 1998, to the hazard of being hit/struck-by
while rotating a large 3000
Lb manufactured steel beam at the number 2 weld station, in that the hoist
was exposed to shock
loading and potential for the hoist braking system failing and the
potential for allowing the beam
to fall.
(a)Among other methods, feasible and acceptable abatement methods to
correct hazards
would include development of operating procedures which prevent shock
loading,
implementation of hoist brake inspections, operators training including
training
verification procedures, and procedures for preventing shock loading of
hoist while
rotating manufactured steel beams in accordance with recognized industry
practices such
as the American National Standards Institute (ANSI) B30.16-1993 Overhead
Hoist
(Underhung).
