recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to injury due to wire rope failure:
a)  Shop #1 - Employees used a two ton hoist to handle material in the
welding area.
The hoist was reaved with 1/4 inch wire rope and had four parts of line.
The wire
rope was damaged in that kinks were readily apparent and one of the
strands in the
wire rope was broken.  This reduced the strength and integrity of the rope
thus
exposing employees to injury in the event of rope failure.
ABATEMENT NOTE:  One feasible and acceptable method to abate this hazard
is to replace
the wire rope on the hoist.  Also, a procedure should be established to
thoroughly inspect the
running rope of all hoists on a monthly basis.

is to
provide a cover over that portion of the foot treadle that is not
protected by the work table.
