General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard when elevated on a pallet on the forks of a powered industrial truck: a) Plantwide - employees were lifted approximately 16 feet off the floor to change light bulbs or perform other functions at that height, while they sat or stood on a pallet on the forks of a Clark Model GCX25 powered industrial truck, serial number GX230-0530- 8380, exposing them to a fall hazard. Among other methods, one feasible and acceptable abatement method to correct this hazard is to secure the platform to the lifting carriage and/or the forks, to ensure the platform has a slipresistant surface, to equip the platform with a standard guardrail (consisting of a toprail positioned with a vertical height of between 39 and 45 inches above the surface of the platform, a midrail positioned about halfway between the platform surface and the uppermost surface of the top guardrail, and a toeboard), to ensure an operator is in the control position of the truck when personnel are elevated, to ensure the truck has overhead protection (and, if necessary, the platform), to provide a means to protect personnel in the normal working position if the moving parts of the truck present a hazard, and to instruct workers to keep their feet on the surface of the platform so as not to negate the protective guardrail system.
