OSHA opens inspections for many reasons — routine scheduling under a national or local emphasis program, an employee complaint or referral, or a follow-up after a reported injury. 5(a)(1)

Serious Gravity 03 1 instance 1 exposed
Issued
Nov 3, 1995
Abate by
Nov 16, 1995
Penalty
Initial $1,500 · Current $500 Reduced
Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
a) Fort Washington Expo Center - Employees used a JLG hydraulic crane
(Model 800
BT, S/N 0408600761) to land material on the roof. The wedge socket end
attachment
of the running rope was improperly applied in that the dead end of the
wire rope was
directly attached to the live end with two wire rope clips. Employees
were thus
exposed to the hazard of a falling load due to failure of the running rope.
ABATEMENT NOTE: Among others one acceptable way to abate this hazard is
to secure
the dead end section of rope by clamping a second and separate piece of
rope to this dead
end.
