employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to being struck by motor vehicles moving in reverse:
a)  Plant - Ready mixed concrete trucks were operated in reverse while
backing into
the loading facility, out of the garage area, and turning around in the
yard.  The
trucks were not equipped with back-up alarms nor were spotters used in the
process
of moving in reverse.
ABATEMENT NOTE:  Among others one feasible and acceptable method to abate
this
hazard is to install back-up alarms on all ready mixed concrete trucks.
