General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by displaced loads due to improper application of wire rope terminations: a) Charging Deck, Ladle Hoist #1 - The running rope for the hoist was improperly terminated at the drum on the left side. A wedge socket was used for termination of the rope. The wedge socket was improperly installed in that the live end of the rope was not aligned with the center line of the pin. Also, the dead end of the rope was attached to the live end using a wire rope clip. b) Charging Deck, Charging Crane - The running rope for the crane was improperly terminated at the drum. Wire rope clips were used for the termination and the clips were placed on the rope such that the U-section was placed on the live end of the wire rope. Among others, one feasible and acceptable method to abate this hazard is to apply the wedge socket and wire rope clips as recommended by the manufacturer.
