5(a)(1):  In that employees were exposed to struck by hazard due to loss
of load:
a)  Employee used two "Renfroe" model WHSR non-locking type clamps to lift
two
one inch steel plates, weighing a total of approximately 13,000 pounds.
The
manufacturer's recommended applications and operations were not followed
in that 1)
the rated capacity of the clamps were exceeded by 1,000 pounds; 2) Employee
attempted to lift plates without edges being even; 3) Handles on the two
clamps were
severely bent thereby hindering their use; 4) The clamps were not held in
place until
the lifting sling was tight enough to maintain the clamps' position on the
plates; 5)
Employee stood approximately eight to ten feet from the plate being
lifted; and 6) An
inspection of the clamps was not conducted before each lift.
ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement
method
to correct this hazard is to strictly follow the manufacturer's
recommendations
and
instructions.
