Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: (a) A below the hook lifting device designed to be used for a vertical lift was used in such a way that a side pull was applied. This side pull caused the beam clamp to slip horizontally causing the load line to strike an employee performing demolition operations at the Port Washington Wisconsin Electric Power Plant Unit #5. The operating practices used with the lifting device did not meet the requirements of ANSI B30.20-1985 Below the Hook Lifting Devices Section 20-1.6.4 Lifting Device Operating Practices. Among other methods, one feasible and acceptable abatement method to correct this hazard is to use the lifting device for vertical pulls only.
