Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential failure of shop-made devices. American National Standard Institute (ANSI) B30.20, Below the Hook Lifting Devices: Section 20-1.2.1, Markings. (a)RATED LOAD MARKINGS. The rated load of the lifting devices (for lifting sand molds and castings), located in Plants #1 and #2, were not marked on the main structure where it is visible. IDENTIFICATIONS. A name plate or other permanent markings were not affixed displaying the following information: 1)Manufacturer's Name 2)Serial Number 3)Lifter Weight, if over 100 lbs. 4)Rated Load Among other methods, acceptable means of abatement would be provision of either permanently affixed tags, or actual markings of the device with data required above.ed

General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to pressurized sand described in ANSI Z241.1. Section 5.3.2.2 Title: Blow Plate and Core box Seal. (a)The "Redford Core Shooter" machine, located in Plant #1, had pressurized air and sand blowing out of the unit at the head flange connection. The flange gasket had failed, and pieces of cardboard were being used as a substitute head gasket to seal the unit. This condition exposed the employees to potential eye injuries by the release of pressurized air & sand and other debris. Feasible abatement and protection can be accomplished by one or more of the following means: 1)By assuring that magazine plates, blow plates, and mating surfaces on core boxes do not become worn to the point where a good mechanical seal can not be achieved. 2)Cleaning loose sand from all mating surfaces before the core box is blown. 3)Use of flange gasket and/or other forms of sealant members between mating surfaces. 4)Protective shields and/or curtains between operator and machine. 5)Maintain the vents, blow tubes, and slots to prevent excessive build-up.
