Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)Employees operating an "Atwood Scale-O-Matic model #S400, serial #1692"
pizza dough machine located on the second floor bakery, were not trained
and/or
provided proper instructions outlined in the "Atwood Scale-O-Matic Dough
Divider and Rounder-Operation/Safety Manual" and found in "ANZI Z50.1 and
Z50.1-a for Baking Equipment/Safety Requirements".  Employees were exposed
to amputation hazards due to the lack of information regarding the
following
elements which cover:
1.  Safety Instructions (Caution, Warning, and Danger signs).
2.  Operating Instructions.
3.  Guide to Cleaning and Maintenance.
" One or more feasible and acceptable abatement methods, among others, to
correct this
hazard is:
To maintain machine and user safety, the responsibility of the owner is to
read
and follow these instructions:
1. Utilize all safety instructions.  Keep them readily available for
machine
operators.
2. Make certain all operators are properly trained, know how to safely and
correctly
operate the unit and are properly supervised.
3. Allow operation of the unit only with all parts (including safety
equipment) in
their correct position.
4. Inspect and maintain the unit on a regular basis and perform all
maintenance
as
required.
5. Keep all instructions available for the operator and insure that all
warning labels
on the unit are clean and readable.
