The hazards of bursting and/or flying components or contents caused by
potential failure
of an unmarked pressure vessel which was not provided with required
safeguards.  The
vessel was not marked or certified under the requirements of the American
Society of
Mechanical Engineers (ASME) Boiler and Pressure Vessel Code, Section VIII,
Rules for
Construction of Pressure Vessels, Division I.  For example:
(a)The Binks Model 83-5700 Pressure Tank used for impregnating castings
did not
conform to the ASME requirements in the following areas:
(1)The pressure vessel was not provided with the markings required by
Paragraph UG-116 of the Code, which includes:
(A)The official code symbol;
(B)The manufacturer's name;
(C)The manufacturer's serial number; and
(D)The year of manufacture.
(2)The pressure vessel was not provided with protective devices, safety and
relief valves or rupture discs, as required under Paragraph UG-125(a) of
the Code.
Among other methods, one feasible and acceptable method to correct these
hazards is to
assure that the design, manufacture, certification, and use of all
pressure vessels is in
accordance with the most current ASME Boiler and Pressure Vessel Code.l
