Section 5(a)(1) of the Occupational Safety and Health Act of 1970:
The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to being struck by the drilling machine's
swivel head and/or being caught between the descending swivel and
the mast on the drilling machine:
a.Employee(s) required to enter the mast on the Walker-Neer
Apache Drill Rig #5, Model 250-40, serial number 2322, to
thread and/or align tubulars (pipe sections) by hand into the
jaws on to the pipe boom, were exposed to the hazard of being
struck by the descending swivel and/or being caught in between
the descending swivel and the mast.
Feasible methods of abatement, among other methods, include:
Prevent employees from entering the mast to thread/align
pipe by hand by following the manufacturer's safe
operating procedures to adapt 3-1/2 inch pipe clamps for
safe handling of 2-7/8 inch OD tubulars; procedures which
would include using pipe clamp reducer inserts necessary
to hold 2-7/8 inch tubulars in alignment with the top
drive quill adapter sub which will negate the need for
employees to enter the derrick mast to hand thread/align
tubulars.ign
