Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and
a place of employment which were free from recognized hazards that were
causing or likely to cause death or
serious physical harm to employees in that employees were exposed to
struck-by hazards:
a.In the enveloping area of the retread manufacturing plant, the employer
did not ensure that employees
involved in repairing envelopes and re-inflating curing tubes to ten
p.s.i. in tires utilizing multi-piece curing
rims conducted the inflation while the tire was laying flat on the floor
and while the employees stayed
outside the trajectory, thereby exposing employees to a struck-by hazard
from flying parts.
Among other methods, feasible and acceptable abatement methods to correct
this hazard include, but are
not limited to, the following:
a.Follow the enveloping procedures in the Bandag instruction manual which
stated the following:
"While the mounted tire is lying flat on the floor, inflate the tube to
ten
p.s.i. maximum (69 kPa)
to firm up the sidewall of the tire and seal the envelope.  Bandag
recommends the use of an air
line regulated to ten p.s.i. (69 kPa) for this purpose.  Then and only
then should the tire be
moved".
b.Require and enforce the repair and inflation of tires utilizing the
multi-piece curing rims to be
conducted while the tire is laying flat on the floor.
c.Require and ensure that the regulator for the air line remains set at no
greater than ten p.s.i.
d.Require and enforce that employees stay out of the trajectory while
inflating tires utilizing multi-
piece curing rims.
e.Provide and utilize restraining devices and barriers for the inflation
of tires utilizing the multi-
piece curing rims.
f.Revise the written enveloping procedures in the Bandag instruction
manual
to include and require
that all repairs and re-inflation of tires utilizing multi-piece curing
rims be performed in a safe
location while the tire is laying flat on the floor and to stress the
hazards if these procedures are
not followed.
g.Coordinate with the manufacturer of the ARC bands which require no air
pressure to design and
fabricate ARC bands for the smaller size tires as a substitute for the
multi-piece curing rims.
h.Provide and document retraining for all employees involved in the
enveloping process.ng
