Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or were likely to cause death or serious physical harm to
employees, in that: the
employees, operating the Ford Tractor model number 445A,  were exposed to
the hazard of
being struck by the bucket which was not properly attached.  The bucket
was missing the pin
attachment bracket that prevents the bucket from dislodging when pinned to
the tractor
attachment plate:
Among other methods one feasible method of correcting the hazard is to
remove the
defective/unsafe equipment from service until repaired.

construction sites
and in the yard, were exposed to being struck by falling objects such as
trees, sand, gravel,
loose equipment parts (bucket, cylinder rods, bolts), etc. when the bucket
was raised during
loading:
Among other methods, one feasible methods of correcting the hazard is to
provide fall
over protection structure canopy with 4 posts as required by the
manufacturer.
